I am aware that participation in any of the Crossed programs has some inherent risks and injury can occur. On rare occasions, these can be serious. In consideration of my child/children being allowed to participate in Crossed programs.

I/ the parent/guardian, assume the risk of all injury and agree not to sue the Crossed trainers, directors, coaches, assistant coaches, or volunteers for any and all injuries cause by or resulting from participating in any Crossed programs.

The Crossed staff are authorized to attend to any health problem or injury my son/daughter may incur while attending any of our programs.

I/We understand that my son/daughter must have current and active medical insurance before he/she may attend Crossed and I hereby confirm that he/she does.

By checking off on this waiver, I also authorize the use of pictures/videos of the above-named participant(s) to be posted on the website or advertising media published by Crossed.
